Refinance/HEL Checklist

1. Copy of Deed;

2. Social Security Number(s);

3. Paid receipts or final bills for real estate taxes, water, sewer and municipal electric, if applicable;

4. Discharges or releases of any encumbrances, other than bank mortgages;

5. Names of Lender(s), telephone number(s), and account number(s) for all outstanding mortgage(s) encumbering the property being sold;

6. Name and telephone number of insurance agent issuing home owner’s insurance, if not condominium;

7. Name of Attorney if represented by counsel;

8. If property being sold is a condominium, a 6(d) certificate issued by the condominium association;

9. If the property being refinanced is a condominium, the name and phone number of a person to contact with the condominium association;

10. If the property being refinanced is a condominium, the monthly condominium fee
